instrument rental agreement
All families must complete and return this Instrument Rental Agreement in order to borrow an instrument from CAMP. All students receive an instrument at no cost for the length of time they participate in CAMP programming. Click here to download.
CPS Permission slip
All students need to complete this form in order to participate in a program located at a CPS school. Click here to download.
breakthrough pick-up release
Students that also participate in Breakthrough’s after-school program may fill out this form in order to walk to Beidler Elementary with a CAMP teacher.
